At BOLD Collective, LLC, your privacy matters. All personal and health information you share with us is kept confidential and protected under HIPAA guidelines.

We only use your information to:

  • Provide therapy, wellness, and consulting services.

  • Schedule appointments and communicate with you.

  • Process payments or insurance claims.

We will never share your information without your consent, unless required by law (such as risk of harm, abuse, or court order).

You have the right to access your records, request corrections, and ask how your information is shared. We use secure systems to protect all data.

Practice Fees

At BOLD Collective, LLC, we believe therapy and wellness should be accessible and transparent. Below is our standard fee structure:

Counseling & Wellness Services

  • Initial Intake Session (60–75 minutes): $130

  • Individual Therapy Session (50 minutes): $130

  • Couples/Family Session (50–60 minutes):$140

  • Group Therapy (per person, per session): $60

Sliding Scale

We offer a limited number of sliding scale spots based on financial need. Fees are adjusted according to income, household size, and personal circumstances.

  • Sliding scale rates range from $60 – $100 per individual session.

  • Proof of income may be requested.

  • Sliding scale spots are reviewed and reassessed every 6 months.

Payment

  • Payment is due at the time of service.

  • We accept [credit card, HSA/FSA cards, cash, and/or check — adjust as applies].

  • Some services may qualify for reimbursement through out-of-network insurance benefits; we can provide superbills upon request.

NO SURPRISE ACT

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

·       You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

·       Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

·       If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

·       Make sure to save a copy or picture of your Good Faith Estimate.
